SUMMARY OF THE PROJECT
Lintervento consiste nella sperimentazione di un nuovo modello assistenziale integrato di continuita ospedale-territorio-domicilio, finalizzato a stabilizzare la situazione clinica di cronicita (soprattutto per i pazienti fragili), evitando episodi di riacutizzazione che comportano spesso il ricovero, lutilizzo di risorse sanitarie in modo improprio e la pressione sulla famiglia del paziente
